Personnel Adjustments
All changes in personnel status must be approved by the appropriate department head and the Dean's office. This chart reflects those University forms that must be completed and submitted. Forms are available in the Business Office.
Hiring of employees is completed by the web-based hiring system.
Two documents must be completed authorizing the department's Payroll representative to be able to access the Student Hire System and input student hires. The departmental Payroll representative will have complete responsibility to enter documents, the Hire and Approval form will need to be completed. The Business Manager and the Associate Dean for Administration will review and approve hires completed by individuals for the department. Only the Business Office or Dean will have authority to approve new hires as specified and documented by completion of the Hire and Approval form.
The New Hire Entry form will need to be completed for the individual authorized to enter the information. A printed version of this form will be forwarded to the Business Office for review and authorization after signature approval is given by the Department Chair. Also, the individual accessing the system must complete the Statement of User Responsibility/Confidentiality Agreement. These documents giving your Payroll representative authority to either initiate hire documents or approve hire documents or both, must be completed before access to the system will be given and training can be completed.
Requests for the hiring of graduate assistants will initially require the submission of the College of Education offer letter prior to input into the web-based hiring system. The top portion is to be completed and signed by the Department Chair and forwarded to the Dean's Office. This serves as the request to offer the assistantship. If approved, the offer letter is signed by the Dean's Office and returned to the initiator for presentation of the offer to the student. The form is then returned to the Business Office with the bottom half completed noting acceptance/rejection of the offer. The web-based entry form accompanies the offer letter if accepted.
