Dual Employment Requests
ALL requests for Dual Employment are to be initially submitted to the proper department chair (if applicable) and then forwarded to the Business Office of the College of Education. IN NO INSTANCES SHOULD THIS PAPERWORK BE INITIALLY SENT TO HUMAN RESOURCES.
Potentially, three forms may need to be completed:
Request for Approval of Dual Employment (P-300)
This form is only to be completed if the individual is an employee of the University of South Carolina. Otherwise, it does not need to be submitted. All sections of this form must be completed, including "Regular Duties", amount, employee's signature, and department head signature.
Dual Employment Request Form (CC-CL-020)
This form must be completed for all individuals requesting dual employment. Again, all sections must be completed, including "service description", dates, times of work, amount to be paid, and signatures. This information must be consistent with the same data listed on the "Request for Dual Employment". The information for the "Employing" (Home) Agency will be completed by the Business Office.
Dual Employment Retirement Information
This form must be completed if the individual is a member of the retirement system, being paid by USC, and employee of another State agency. This tells USC to deduct retirement contributions from their dual employment pay.
