Dear Parent/Guardian:

To complete requirements for EDEX 790: Introduction to Assessment, graduate students evaluate individual children on academic skills. This will be done through the administration of the Woodcock-Johnson III and two follow-up assessments based on the results from the Woodcock-Johnson III. Your child has been selected to participate in this project. We are requesting permission to have your child evaluated by a graduate student in the EDEX 790 course. All information on the results will be kept confidential. You can have access to the results and you can request that the results be kept confidential from the school. If you agree to have your child assessed using these tests please sign and return the bottom portion of this letter with your preferences for results. If you have questions or would like more information, please call me at the special education office 777-5734 or send an email cwissick@sc.edu
